Add Violation ID column to view_instances.php report

The request is to add the violation id (located at the end of a url when viewing a best practice.  ie: https://amp.levelaccess.net/public/standards/view_best_practice.php?violation_id=338 has a violation id of 338) as a toggleable option when generating reports.
